OK, here's one. A throws to B, when d(efender) turns around, B throws it back and A throws it at D's head, then when D turns around he passes it to B and B throws it at his head. There's another one I'm not so sure, a guy behind the defender covers his head and then the guy with the ball throws it off, but pretends to keep dribbling.
